Privacy Policy
While providing nutrition services, Healthiest Version of You will collect, store, disclose and access a range of health-related information about the Participant that may be considered sensitive. Healthiest Version of You will only collect information that is necessary to provide the highest quality of health service and the nutritional services as outlined in this Service Agreement. The Service Provider may disclose information to the Participant’s treating doctor and/or health specialists from whom they are currently receiving treatment. If the Participant does not want their health information to be provided to those health providers or the NDIS, the Service Provider may not be able to offer services to the Participant. In accordance with privacy legislation, the Participant is entitled to access any information that Healthiest Version of You holds about the Participant.
The Service Provider may need to disclose information about the Participant without consent from the person involved. This might include mandatory reporting requirements on child protection matters, and obligations to report incidences of violence, exploitation, neglect and abuse, and sexual misconduct to the NDIS Commission and police.
